In-House Plan

At LP Dental & Cosmetic, we are committed to providing you with effective dental treatment at our practice. We are proud to provide all patients with our LP Discount Membership Plan. It should also be noted that our in-house plan is a savings plan and is NOT treated as insurance. This is a great benefit for patients who receive care only in our office!

Preventative Program: Patients without Periodontal Disease (2 dental cleanings a year)

• Per Person: $299/year
• Per Couple: $499/year
• Per Child (less than 12 years old): $249/year
• Per Family (Maximum 2 adults and 2 children below 18 years of age): $899/year

Periodontal Program: Patients with Periodontal Disease (Up to 4 periodontal maintenance appointments per year)

Add $299 per person to the Preventative Program

Includes:

• 2 prophylaxis (dental cleanings)
• 2 exams
• Unlimited PA x-rays
• 1 fluoride/varnish treatment
• 4 bitewings x-rays
• 1 adjunctive oral cancer screening
• 20% off ALL standard diagnostic, preventative, restorative, and prosthodontics fees
• 15% off ALL standard endodontic and periodontics fees
• 10% off ALL standard implant and surgical fees
• 10% off ALL standard orthodontic and Invisalign fees

Guidelines

• Plan is valid for 12 months from the original date of purchase
• The next periodontal or preventative appointment MUST be scheduled before you leave the office. If you need to reschedule or cancel, please call the office at least 36 hours prior to your appointment.
• It is the responsibility of the patient to schedule your appointments and ensure that they fit within the 12-month plan period. If you do not make appointments in that period, you automatically forfeit ALL benefits of the plan.
• NO CARECREDIT ALLOWED TO PURCHASE THE IN-HOUSE PLAN
